  1. HEALTH CARD OPTIONS
  2. The individuals in the age group of 06 months to 65 years are eligible to opt for one of the following:

    HEALTH PLAN (HP): This Health Card will cover Health Check-UP worth Rs 2100/= as per the list provided. It also offers the facility of Cashless Hospitalisation up to Rs 50,000/= through nationwide network of leading hospitals.

    Annual Charges: Rs 1950/=.

    10% extra will be charged for people above the age of 55 years.

    HEALTH PLAN PLUS (HPP): This Health Card will cover Health Check-UP worth Rs 2100/= as per the list provided. It also offers the facility of Cashless Hospitalisation up to Rs 1,00,000/= through nationwide network of leading hospitals. Annual Charges: Rs 2950/=.

    10% extra will be charged for people above the age of 55 years.

  3. VALIDITY OF THE CARD
  4. The Health Card would be valid for a period of 12 months.

    The validity period will start from 1st of the following month after the proposed card holder has gone through the process of health checkup.

    The company / Insurance company reserves the right to accept or reject any of the application without assigning any reasons.

    It will take approximately 3 to 4 weeks time for issue of health card. The Health Card will be valid only at panel hospitals. The list of hospitals is continuously being amended from time to time and, hence, is subject to changes.

  5. HEALTH CHECK-UP INCLUDED IN HEALTH PLAN
  6. Checkup worth Rs 2100/= are inbuilt in the Health Plan (HP) and Health Plan Plus (HPP). The following is the list of Health Check-ups, which every individual cardholder will have to undergo. The preventive health checkup will include the following :

    Consultation with the Medical Specialist/ Gynecologist.

    TMT or Mammography

    Ultrasound Abdomen

    X-Ray Chest

    Urine Routine Examination

    Blood Tests to include Hb, Blood sugar, Cholesterol and Triglycerides, Creatinine and SGPT (for men) OR Pap Smear (screening for cancer cervix) for women.
